I believe a yellow tag is the FAA 8130-3 form. The FAA does not
require this, even though many people say it does. Any paperwork which
contains traceability information is sufficient. I can dig out the
reference for this.

But if you have your own radio repaired, there is no need for a new
traceability do***ent.

Lots of US firms cannot issue 8130-3 forms. I believe you either need
to be an FAA Repair Station, or you need to rent-in an inspector :)
